Heather Leisy, MD, MBA, MPH is the Medical Director of Population Health Quality in the Office of Population Health and Accountable Care department. Before taking her role in OPHAC, she served as the Director of Preventive Care in Federally Qualified Health Care Center. She completed her medical degree from the Medical University of South Carolina, residency from Stony Brook University. She is board certified in General Preventive Medicine & Public Health and Clinical Informatics. She also holds a master's degree in business and public health. 

Title of Talk: "Incorporating Remote Patient Monitoring for Chronic Disease Management: A Hypertension Program at UC Davis Health"

Our remote patient monitoring program for hypertension supports quality improvement and patient-centric care by extending blood pressure management into the home setting under an interdisciplinary team. Using connected blood pressure devices, lifestyle and medication management, standardized monthly visits, and patient education, our RPM program engages patients with near real-time support to control this disease. This preventive medicine approach strengthens clinical care, reduces barriers to access, and advances population health goals by improving blood pressure control which is associated with reduction of long-term cardiovascular risk.
